Important Information

Attention IN Agents and Policyholders:

The state of Indiana Department of Insurance has imposed a moratorium on policy terminations in the counties impacted by recent storms and floods. Policyholders in the affected counties can not have their coverage cancelled due to non-payment of premium between 5/31/08 - 8/16/08. Accordingly, First Chicago Insurance will be suspending cancellations for non-payment by extending the cancellation date in these areas to August 17, 2008 at 12:01 a.m.

Billing statements will continue to be mailed each month. Policyholders can opt-out of coverage by sending a signed Cancellation Request form to our Home Office. If the policyholder does not opt-out of coverage, they will be required to pay their earned premium by August 17, 2008 in order to avoid a lapse in coverage.

Impacted Counties: Adams, Bartholomew, Benton, Boone, Brown, Carroll, Clay, Daviess, Dearborn, Decatur, Elkhart, Franklin, Fulton, Gibson, Greene, Hamilton, Hancock, Henry, Huntington, Jackson, Jay, Jefferson, Jennings, Johnson, Knox, Lagrange, Lawrence, Madison, Marion, Martin, Miami, Monroe, Montgomery, Morgan, Newton, Ohio, Owen, Parke, Pike, Posey, Putnam, Randolph, Ripley, Rush, Shelby, Sullivan, Tippecanoe, Union, Vanderburgh, Vermillion, Vigo, Wabash, Warrick, Wayne, Wells, White.

Accordingly, First Chicago Insurance will be suspending cancellations for non-payment by extending the cancellation date until after August 18, 2008.

Billing statements will continue to be mailed each month. Policyholders can opt-out of coverage by sending a signed Cancellation Request form to our Home Office. If the policyholder does not opt-out of coverage, they will be required to pay their earned premium by August 30, 2008 in order to avoid a lapse in coverage.

Impacted Counties: Adams, Calhoun, Clark, Coles, Crawford, Cumberland, Douglas, Edgar, Hancock, Henderson, Jasper, Jersey, Knox, Lake, Lawrence, Madison, Mercer, Monroe, Pike, Randolph, Rock Island, St. Clair and Winnebago.
